Summary. Through the ten-years clinical history of a young man afflicted with a serious disturb in the psychiatric area, the authors describe the various stages of the therapeutic project which is developed with a bio-psycho-social-cultural model. It goes through a long pathway that involves different therapeutic agencies articulated in a network of public, private and university services. The attention is focuses on making the “madeto-measure clothes” for the patient in a continuous effort by the therapists to catch the needs of the present, in addition to a continuous consideration on the possible developments. Particular attention was turned to the familiar and environmental contest through different cultures mediation from the work groups and from the various involved agencies.
